To Be Noted:
Crusaders FC reserves the right not to register a player unless all the following have been complied with:
- All Crusaders FC members (existing and new) must complete a new Registration Document in full each year, before being registered by the Club. MB* There will be NO exceptions to this requirement.1. - All new players transferring to Crusaders FC from another club must provide Crusaders FC with a ‘Clearance Certificate’ from their previous club indicating that the player has honoured all his/her financial obligations to his/her previous club and is free to register with another club.
- All new players must provide Crusaders FC with two irecent passport size photographs (preferably in colour) and a certified copy of the player’s Identity (ID) Book.
- Players will not be able to attend any training sessions nor represent Crusaders FC in any football matches of whatever nature, nor will receive any Club match kit, until full payment of the Club Registration Fees has been received by the Club in advance. Payments (for which proof of payment is required before a Club receipt will be issued) can take the form of either a Cash payment, a Cheque payment, a Credit Card payment (where this facility is offered), or a Direct Transfer or Deposit into the Crusaders FC Bank Account, details of which are set-out below. NB* Club Registration Fees, once paid are strictly NON-REFUNDABLE. Please note that there will be NQ exceptions whatsoever to this proviso.
- Parents and Players (and anyone else in their company) are expected at all times to adhere to the Crusaders FC Code of Conduct and all Club Policies, Rules & Regulations (as amended from time to time).
- NB* Where a player is 19 (nineteen) years of agIe or younger but will represent Crusaders FC in the Senior League Section, such player will need to complete a Crusaders FC JUNIOR LEAGUE SECTION REGISTRATION DOCUMENT as consented by his/her parent(s)/guardian(s).
Players’ Declaration
- I understand that the Club reserves the right to reject and/or terminate any membership at any time, and that all memberships need to be ratified by the Club’s Executive Committee each year.
- I being the player whose details appear herein accept the constitution and by-laws, code of conduct and general policies of the Club (“rules and regulations”) as amended from time to time, as applicable.
- I agree to abide by and comply with all these rules and regulations and any other rules and regulations that may be adopted by the Club from time to time, and will have no recourse whatsoever against the Club in this regard where such rules and regulations are enforced against me in any way.
- I hereby acknowledge that it is incumbent upon me to make myself and anyone else in my company familiar with all the rules and regulations pertaining to the Club.
- I acknowledge that I am fully responsible for myself whether on the Club premises (i.e. currently the St. Stithians College Campus) or otherwise, before, during or after any Club match/activity/event/function/involvement of whatever nature.
- I acknowledge that the Club does not take any responsibility whatsoever for any theft or loss of, or damage or destruction to, any item or property of whatever nature brought onto the Club premises by me or anyone else in my company.
- I hereby assume absolute responsibility for the payment of any charges, costs or fees whatsoever due and payable to the Club by me and that no indulgence or grant of time by the Club in this regard shall be deemed a waiver of the Club’s rights to reject and/or terminate any membership granted, nor recover such amount(s).
- I confirm that all the particulars that I may furnish or that have been furnished on any Club forms shall, to the best of my knowledge and belief, be full, true and accurate and I undertake to advise the Club in writing of any changes to any details included therein.
- I do hereby declare that I fully understand and accept ALL the provisos contained herein in this form.
Player Consent & Indemnity
I, the registered player, being a player at Crusaders Football Club (hereafter referred to as “the Club”) hereby request/give permission for myself to participate in Football and other associated activities (“activities”) at the Club. I accept that all reasonable precautions will be taken by the Club to ensure the safety and welfare of myself and that of my belongings but also acknowledge that the Club has made no undertaking(s) whatsoever to me in this regard.
Furthermore, I hereby undertake not to make any claims against the Club, it’s officials or agents and hereby indemnify all of the foregoing against and absolve them absolutely from any claims, notions, suits, procedures, costs, expenses, damages and liabilities of whatsoever nature including lawyers fees I may incur in respect of any loss, damage, injury or death to any person(s) and to any property arising out of or in connection with or resulting from the above mentioned acitivities.
I hereby also accept that I shall be responsible for payment of all medical and/or hospital accounts, where applicable, should injury be sustained by me resulting out of any activities undertaken at/on behalf of the Club.
I undertake to participate in such activities only in accordance with the safety rules and suggestions presented to me by the Club. I understand and accept that intense physical and mental exertion may occur during the course of such activities and therefore I further warrant that to my knowledge, I have no medical or mental condition or complaints that in any way may end anger myself or any other persons during or after participation in such activities and that I am in good and sound health.
I have disclosed under the Medical Information Section on page 3 of the Registration Document, aspects about myself that the Club officials should be aware of (e.g. allergies, other medical conditions etc.) as well as relevant information that is essential in case of a medical emergency or a requirement for medical treatment or hospitalisation. I have read and fully understand and accep the content set out herein.
Parents’/Guardians’/Players’ Declaration

- I/We agree to abide by and comply with all these rules and regulations and any other rules and regulations that may be adopted by the Club from time to time, and will have no recourse whatsoever against the Club in this regard where such rules and regulations are enforced against I/we/us.
- I/We hereby acknowledge that it is incumbent upon me/us to make myself/ourselves and anyone else in my/our company familiar with all the rules and regulations pertaining to the Club.
- I/We acknowledge that I/we am/are fully responsible for my/our child whether on the Club premises (i.e. currently the St Stithians College Campus) or otherwise, before, during or after any Club
match/activity/event/function/involvement of whatever nature.
- I/We will ensure that the player referred to herein is made aware and abides by all the rules and regulations applicable to him/her.
- I/We acknowledge that the Club does not take any responsibility whatsoever for any theft or loss of, or damage or destruction to, any item or property of whatever nature brought onto the Club premises by either me/us or my/our child or anyone else in my/our company.
